WHAT IS YOUR NEW PATIENT POLICY?

First time patients are asked to arrive 30 minutes EARLY to allow adequate time for completing the initial paperwork. For purposes of maintaining continuity of care, we ask that you bring us the latest relevant records with the most recent test results and current medications list.

WHAT IS RURAL HEALTHCARE?

In medicine, rural health or rural medicine is the interdisciplinary study of health and health care delivery in rural environments. The concept of rural health incorporates many fields, including geography, midwifery, nursing, sociology, economics, and telehealth or telemedicine.

WHAT IS PATIENT-CENTERED MEDICAL HOME (PCMH)?

A care delivery model whereby patient treatment is coordinated through their primary care physician to ensure they receive the necessary care when and where they need it, in a manner they can understand.

WHAT IS YOUR CANCELLATION POLICY?

In order to be respectful of the medical needs of other patients, please be courteous and call
Total Family Medical’s office promptly if you are unable to show up for an appointment. This time will be reallocated to someone who is in need of treatment. If it is necessary to cancel your scheduled appointment, we require that you call at least 24 hours in advance.

Appointments are in high demand, and your early cancellation will allow another patient access to timely medical care.

HOW DO I CANCEL MY APPOINTMENT?

To cancel your appointment, please call 985-542-2466. If you do not reach the receptionist, you may leave a detailed message via voice mail. If you would like to reschedule, please leave your name and phone number. We will return your call promptly.

A cancellation is considered to be late when the appointment is cancelled without a 24 hour advance notice.

CALLING AFTER HOURS?

If it's an emergency, please hang up and dial 911. If it is not an emergency, please call (985) 542-2466 and leave a detailed message including your name, number and the reason for your call. If your situation is URGENT, someone will get back to you within 20 minutes. If it is not urgent someone will contact you the following business morning by 9:00 a.m.
